Introductions

Anyone can make an introduction to our service. If you feel a child or young person who has a diagnosis of Autism and/or a Learning Disability is at risk of a Tier 4 hospital admission, you can self-refer to our service by following the steps below.

We work with young people and their families during times of increased complexity or risk. Our support is shaped by the level of risk identified, and is in place only while those risks are being actively managed. All decisions are made through a multi-disciplinary team (MDT) to make sure that the right support is offered at the right time. As risks are reduced or circumstances change, we work together to plan a safe and supported step-down.

When a young person is supported by our service, they sit on the Dynamic Support Register (DSR), which allows us to connect with different specialist agencies who may be involved in the young person’s care and support.
