The Keyworker Collaborative History

The Keyworking Function has been developed as a response to the NHS England and NHS Improvement Long Term Plan commitment that by 2023/24, childrenand young people with a Learning Disability (LD) and/or Autism, with the most complex needs will have a designated Keyworker. Our collaborative has grownand developed over the last few years to form what the KWC is today. Our host organisations, The Edmund Trust and Circles Network, have workedcollaboratively for over 25 years, and continue to strive for transformational change together.​

PATH

PATH: Planning Alternative Tomorrows with Hope

PATH is a Person Centred Planning (hyperlink to Person centred planning page) tool that is used to plan goals and dreams for the future and work out theactions that need to be taken to achieve those goals. Each year, our team comes together for a day to identify and plan the steps we will take each quarter toachieve our targets as a collaborative. Below is our most recent PATH: a snapshot of our ongoing story as a service supporting young people.​

Midnight, Dawn and Midday

When the pilot project for the Keyworker service launched in Peterborough and Cambridgeshire in 2021, the development team recognised the importance of involving experts with lived experience. Their voices were vital in shaping the service and offering meaningful insights into the challenges faced by people with learning disabilities and mental health conditions.​

As part of this commitment, the team employed two young people—one of whom is Daniel Paice. Daniel is a talented author and poet who has published a book titled Prevailed Hope. Featured in the collection are the poems Midnight, Dawn, and Midday, each written during Daniel’s personal journey with mental health.
